Forge Dojo Martial Arts Policies

1. Membership Terms

  • Membership is billed monthly, with fees automatically deducted on the same date each month as the initial registration.
  • Membership fees are non-refundable, except under specific circumstances outlined below.
  • Membership includes access to all scheduled classes for your skill level as outlined in your membership plan.

2. Cancellation Policy

  • Memberships can be canceled at any time with a 30-day notice before the next billing cycle. To cancel, notify us via email or in person at the dojo.
  • Cancellations requested less than 30 days before the next billing date will take effect the following billing cycle.
  • No refunds are issued for partial months or missed classes.

3. Suspension and Freeze Policy

  • Memberships can be suspended or "frozen" for up to two months per calendar year for medical or travel reasons with appropriate documentation.
  • To request a freeze, notify us at least 14 days prior to the desired start date.
  • During the freeze period, members will not have access to classes and billing will pause temporarily

4. Class Attendance and Conduct

  • Members are expected to attend classes regularly and arrive on time.
  • Proper conduct and respect for instructors, fellow students, and the dojo facilities are always required.
  • Forge Dojo reserves the right to revoke membership without refund if a member exhibits inappropriate behavior or violates dojo policies.

5. Liability Waiver

  • Martial arts training involves physical activity, and participation is at the member’s own risk.
  • Members must sign a liability waiver before attending their first class, releasing Forge Dojo, LLC, its instructors, and staff from responsibility for any injury or harm that may occur during training.

6. Changes to Membership Terms

  • Forge Dojo, LLC reserves the right to modify membership terms, fees, class schedules, and dojo policies with 30 days' notice.
  • Notice of changes will be provided via email and posted at the dojo.

7. Payment and Late Fees

  • Membership dues must be paid by the due date each month. If payment fails, a late fee may apply.
  • If dues remain unpaid for more than 15 days, membership access may be temporarily suspended until the balance is cleared.

STUDENT POLICY WAIVER

All students that are taking a COMPLIMENTARY class, as well as new students that are enrolling in a monthly membership, are required to complete the Student Policy Waiver form in order to participate in any classes at Forge Dojo.
